Healthcare M&E carries a statutory layer that ordinary buildings do not: medical gas pipeline systems with their alarms and manifolds, critical ventilation with defined pressure cascades for clinical areas, and essential power that must transfer without an interruption a patient would notice.

The constraint that shapes everything is that the building never closes and the occupants cannot easily be moved. Redundancy has to be real rather than nominal, and maintenance routes must not require entering an occupied ward — decisions taken at layout stage, because they cannot be retrofitted later.
